Do you need to report state disability insurance? My wife was on disability for about 8 weeks due to pregnancy. We have a tax appointment this weekend but I have not received the 1099G form for disability only Family leave act. Will she get the 1099 or not? I looked on the SDI for california and it does not mention anything about having to report it. Paid Family Leave does say that the money will be reported and you will get the 1099G but disability does not mention anything. Before I call my tax guy or the state does anybody know anything about this?
IRS tax deductibility of SDI, State Disability Insurance, and VDI, Voluntary Disability Insurance? Two questions: 1) Many companies require employees to pay State Disability Insurance, SDI, while other require Voluntary Disability Insurance, VDI. It appears that SDI payments are tax deductible on the Federal return but VDI are not. Why is this? 2) Are benefits paid under each of these plans taxable or non taxable? thanks
Is W-2 Box14 entry for CASDI (California State Disability Insurance)a valid claim on Form1040 Sched A Line5? Instructions for Line5 say to include "Mandatory contributions made to California, New Jersey, or New York Nonoccupational Disability Benefit Fund, Rhode Island Temporary Disability Benefit Fund, or Washington State Supplemental Workmen's Compensation Fund" which seems vague at best. I have scoured the web for information addressing this question. CASDI is certainly mandatory.
Where do I pay the State Disability Insurance? I just joined the company as a Book-Keeper. We use ADP service for Payroll. In ADP Report there is deduction from employee paycheck for 1.20 per employee. ADP pays all the other taxes but this comes as a client responsibility. I just want to know whom should I pay this SDI deduction to? We are based in NY.
State Disability Insurance in California? I was laid off in December (company went under) and in March I got pregnant. I have not found a permanent job and at this point am not going to try - who's going to hire a pregnant woman, right? I am receiving unemployment which will be running out in a few weeks and I'm thinking about working for a temp agency until the baby is born to bring in some money. Does anyone know if I am working temp can I collect State Disability Insurance once the doctor tells me no more working? I'm really pissed...worked all those years and now I may not be able to collect through no fault of my own. Any help is appreciated. I think if I'm not working at all I can't collect but not sure. Thanks.
Is this taxable income? State Disability Insurance and Paid Family Leave? I am a little confuse as to which one is taxable income I was on maternity leave for the first two months of 2008 which I collected income from the state (California) and after that time limit was up I collected from the Paid Family Leave Program.....are these both taxable income and if so is it FEDERAL, STATE, or BOTH Please don't refer me to the IRS website unless you have a specific link because I have been there and can't really see a answer Also pls don't tell me to ask my tax professional because I do my own taxes
California State Disability and Insurance? Hi, I am trying to figure out how much of a % I will be getting back from the state. I have short term disability but I havent called them yet to figure out how much I will get every month. Also, I have insurance through work and apparently not all the hospital fees are paid for. For example, my coworker just had a baby and cost her about $1500 out of her own pocket. Altogether was $9000 (rest was billed to the insurance). My doc is telling me I might have to have a C-Section. How much do those usually cost? What the heck is the point of having insurance if there not even going to pay for crap! BTW I am 29 weeks
Returning to work after California State Disability Insurance ends...? My SDI ends in 2 weeks. My boss continued to pay me for 3 weeks after I became ill. I did not ask for this. I explained to her that I did not need the money since I was applying for disability benefits and I am fortunate enough to have an emergency savings account to support myself. Now she wants me to return to work when she's ready since she has to figure out MY schedule after 10 years of service (I have always worked an 8 hr shift 5 days a week) AND the schedule of a “temp” she wants to keep, and my 2 co-workers. I know by staying home an additional 3 weeks it would stop any kind of payback to her which makes since. Can any of this jeopardize my employment? What are my rights?

California State Disability and Private Long Term Disability? Can you collect state and private disability at the same time. My mother is current on state disability for a terminal illness. She also paid for private long term disability insurance. Her private insurance company says that she can not collect both, but I haven't found anything that states that. Does anyone know?
Can I temporarily live out of CA while collecting short term disability checks from SDI California? I just had back fusion surgery and will be out of work for a few months. Is there any rule to prevent me from recovering in another less expensive state while still collecting State Disability Insurance payments? I will eventually be returning to work at my original job in California. Someone told me I must stay in California to continue qualifying for SDI. Any help will be appreciated.
Are rates for disability insurance different in different states? To clarify: I own Guardian own-occupation disability insurance, which was initiated in California. Given a recent income increase, I am eligible to exercise a rider to increase my disability coverage. Are my premiums going to go up a different amount if I increase my coverage through my agent in CA than if I increase it through a different agent, say, in Ohio? Are these riders and insurance premiums nationally uniform for personal disability insurance plans through Guardian?
How do I apply for disability in the state of Minnesota? I go to therapy for generalized anxiety disorder and social phobia, my mom's insurance deductible is almost $600 a month and she makes $1,163 a month. and our rent is $900 a month, how can I go about applying for disability, or different insurance in the state of Minnesota. I'm going to therapy so that I can get a job, but until I finish my therapy I can't do that. Basically I'm looking for website links, phone numbers, or names of organizations. Thanks.
California State Disability for Aspergers Syndrome? Hello. I have Aspergers Syndrome and it is not a real "mental disorder". It is a neurological disorder of the brain. People with AS -their brians are wired differently. I need to find out if people with AS in the State of California can claim disability. I am not talking about SSI or Social Security Disability Insurance. That is the Federal level and I am only asking about the state level. AS is a high functioning Autism and I don't do well socially. I guess you can call it a "social difficulity" or a "social impairment". I was working 72 hours every 2 weeks-on call at Oak Grove, but now they don't want me working there-since I flipped out due to change. People with AS flip out many times, due to change, since people with AS like a lot of routine. Since I am on umenployment-it is just not enough to live on $ 320.00 a month. I was bringing in about $ 980.00 a month. It wasn't great, but shoot-at least I was able to pay most of my bills. Now, I am struggling to pay my phone bill.
is it legal to fire someone on state approved short term disability? was in outside sales. w/ documented depression (not faked). company kept contacting me about doing paperwork so THEY can say i'm on disability - i "had" to get approval from their insurance company UNUM. my doctors were not as diligent as i wanted them to be and they missed a deadline, so the company said that because i missed this deadline, i was terminated as my leave could not be substantiated by UNUM. (1) should they have even been asking me to do all that ? especially since i WAS ALREADY on state disability? (2) do i have to be considered on disability by the company too? as i understand, it's illegal to get benefits from your company AND the state. had the company been able to "substantiate" my claim for disability, that means that i would've gone to jail - right? thanks in advance
does a disability insurance work if I live outside the United States? I am planning to move out of the US back to my home country. My friend tells me that it won't work outside the country, but I think otherwise. Do any of you know if I live outside the United States will I be able to receive a direct deposit or something like that? I am a reliance standard user.
I have an S-Corp with employees, am I eligible for State Disability benefits? I am not an employee of my corp. I own an s-corp with 4 non-exempt employees. But I am not an employee, I am only a 100% share holder and owner of the business. I do not take any salary. Recently I had a baby. Am I eligible for state disablity insurance benefits and FMLA?
Where is the best state and city in the US to live, if one has no insurance but has some medical problems? Also, no disability insurance, but enough income to live comfortably if careful with money? Not enough money to purchase so-called 'high-risk' insurance, but enough to buy needed medicines for a chronic condition, the kind insurance companies refuse to issue policies because of? I know that Texas is NOT the place, but it's where I live, and I'd like to move without making more mistakes. Somewhere nice for older, retired folks would be a boon.
How much monthly benefit do you need when applying for a disability income insurance? I was told as an example, if I gross $6000. Take 80% of that and subtract $2100 to get $2700. The $2100 is the monthly disability benefits I would get from my state (NJ). True?

Disability Insurance in Vermont? Does anyone know if it's legal in the state of Vermont to employee people without disability insurance? I know it's illegal in New York. I'm out of work due to pregnancy complications for 3 weeks or so and I can't collect disability for this or my maternity leave because they said they don't have disability insurance. I called a lawyer, but haven't heard back from them yet. Just curious if anyone knows the answer to this question. Thanks I don't have paid maternity leave, or disability... not lucky at all.

Employer behind on disability insurance payments? I suffer? I recently (4 weeks ago) went out onto maternity leave. I had all of my disability forms in order and sent in. There were no issues with any of them. Yesterday I got a letter from my disability company stating that they were not able to fulfill my claim due to an issue with my employer. I contacted my employer today to find out that they were way behind on payments to the disability insurance provider, and that they were working to come up to date on payments. So here I am 4 weeks into my leave without two dimes to rub together, is there ANYTHING i can do?

Let me state this clearly: I do NOT want to have a child to GET more money-- please let this be understood. My concern is, I am already struggling with money, and when my husband and I decide to have children, I don't want to have to worry. Can an employers insurance company deny longterm disability insurance to a hearing impaired person? My daughter is being denied longterm disability insurance at her place of employment because the insurance company stated her hearing impairment does not fall within their guidelines. I was under the impression this was in violation of the American with Disability Act. Any info provided is greatly appreciated.
Ballpark estimate of disability insurance premiums? My husband's employer does not offer disability insurance and we are looking at getting a simple disability plan from our private insurer State Farm. I would just like to know roughly how much premiums are for this kind of dis. insurance. I've looked online, but you have to enter all kinds of personal information before any company (whether insurance or quote finder) will send you any info. Seems like putting a big sign on my email and screaming "SPAM ME!" So just curious what other people pay for this. My husband is 28, non-smoking, not overweight, and is healthy. He's a high school teacher.

Is there a disability law in NY how many weeks out of work after a C-section? My disability insurance is only paying for 6 weeks after my c-section. My doctor says I should get 8 weeks. Does anyone know of a law in new york state that might support this? I would like to appeal their decision.
On disability - would like to work a few hrs a week. Am I likly to lose $$ I now get from a disabil insur pol? I'm on social security and if I were to put all my "feeling good" time together I could probably work 8-10 hours a week on an irregular basis, teaching computer classes. In my state you can make an average of $800/month while on social security. I also live off of $800 I receive from a disability insurance policy I had which I don't have to pay taxes on. Is it likely that if I were to work 8-10 hrs a week I'd lose this $800 insurance policy? I've been told that with social security if I CONSISTENTLY made $800/month working I'd probably lose my social security but there is no way I would be able to work on a CONSISTENT basis which is roughly defined as 8 hours a day, 5 days a week, but I'm worried about losing the $800 from the insurance company since I probably would not be able to work for that much money. I'd like to work but don't want to shoot myself in the foot? Help?

Employer terminating job while on temp. Disability in NJ? I am out of work on my doctors recommendation and on NJ State Temporary Disability Insurance. After being on medical leave for 3 wks. my employer Terminated my employment indicating I have exceeded my allowed amount of time off. They make it sound like I am on vacation or something! They also indicated I should contact them them upon my release back to work for a list of open positions available with them. This seems illegal from any research I came upon within NJ state guide lines. Is this legal for the employer to do?? It seems to me I was terminated so they are not responsible to pay for part of my Medical Insurance. Any info will be greatly appreciated. "C" in NJ.
If after-tax LTD disability benefit is tax-free, how can employer assess tax on offsets like the CA VDI? I am approved for LTD. According to the cafeteria plan I signed up for, the disability insurance (LTD) benefit should be 60% of my annual salary. Since the employee deductions for the insurance were paid by me with after- tax dollars, the benefit should be tax-free (that is, non-taxable). However, now my employer is offsetting a CA VDI short-term disability benefit against it to the tune of 55% of annual salary for the next year and fully taxing (federal, state, and Social Security) that benefit. I also had to apply for Social Security disability. If I am approved for that, the benefit will be taxed. So my 60% "non-taxable" benefit is shrinking to about 40%. This seems very wrong to me. Any advice? Thank you.
Purchasing insurance to cover maternity leave in California? Is there any sort of insurance you can buy in California so you can get approximately your regular wages during your unpaid maternity leave? I've heard that state disability doesn't pay too much. And how long can you take off? Some women say they've gotten 3 months off but I don't know what I need to do to get that, and still have approximately my regular income coming in. I'm not pregnant yet, so I could buy something without having a pre-existing condition. Thanks!
Have you ever been on workers comp. or state disability? I was injured at work in 2004 and was immediatly placed on temp. dissability pending treatment,I was told that i couldnt recieve state disability cuase i was on workers comp.even though there was money available in my s.d.i Ive had two surgerys and tons of phyisical therapy since then with little improvment my doctors now tell me i need another surgery and even then, they say i can never return to my line of work, Now my temporary dissability has ended due to california state law and im told that i cant apply for state dissability because i havent worked in the last 3 years.Ive worked my whole life and have never been involved in any type of dissability yet im not recieving any income and cant return to work cause my doctor wont authorize it and have been told by my attorney that if i go and get another job it will effect my case against the insurance company,So my question is if there are any other people who have gone through this situation and what did you do for income?need help Does this make sense?
Does anyone in GA know of any unemployment/short-term disability insurance for high-risk pregnancies? I am 24 weeks pregnant with my second child and because of some complications and the fact that my first child was born premature, I am now considered to be high risk. I work full-time right now, but I am not going to be able to work much longer because I am supposed to stay off my feet per the doctor's orders. I think I have heard of programs or insurance for situations like this somewhere, but from reading on the state's department of labor website, it does not seem like I qualify for unemployment insurance because I am leaving my job by my choice. Has anyone been in this situation before? Is there any program or service or insurance that might work for me? It is just not an option for my family right now to not have some sort of second income coming in. I really appreciate any advice you guys may have. I have always worked full-time and had money coming in, and this has me scared! BTW- The company I have worked for for the past year only has 8 employees (so no FMLA)
Question about maternity leave in California? I live in California. I am 31 weeks pregnant and I am suffering back pain. My boss of 3 years is also being horrible to me despite the fact that I have never missed a work day during my pregnancy and I still do my job. I would like to leave work at 36 weeks, but I thought you only get the 4 weeks of disability pay before your due date if your doctor takes you out of work for medical issues. What if my OB thinks I can sit at my desk for 8 hours a day and be okay working full time? I am hoping she will put me on leave early because of back pain and stress from my horrible boss, but if she doesn't then I can't leave early with SDI, right? In California I qualify for the following: 4 weeks partially paid before due date (State disability insurance - SDI) Is this only if my doctor takes me out of work? 6 weeks partially paid after your baby is born, 8 weeks with c-section (SDI) Another 6 weeks of Paid Family Leave (FMLA) Any advice?
State Disability in Florida? Some told me - he had a workman's comp case. Then was issued Social Security and given 100% Disability. Under the Disability, he is not entitled to Medicare for 2 years. It makes no sense to me that if someone was on Disability, they could not get health insurance for 2 years... He claims all his medical expenses are coming out of his pocket. Could this be correct?

Looking for good disability provider in terms of not having to sue them to get a claim.? Okay, I want disability insurance. I realize the value. However, I know that most DI providers will use underhanded tactics not to pay claims - losing paperwork, stating no longer disabled without evidence, stopping payments without explanation, etc... In the meantime, they hope the insured gives up or dies. If I am disabled, I want an insurance company that I can trust not to add that additional burden, or if I am mentally disabled, my family. However, I don't know if there is such an insurer out there. Are there ANY good Disability Insurers in terms of claims responsiveness? Is there any post-claim customer satisfaction rating, or any other independent gauge I can use, other than just trusting the selling agent? -->Adam Source: http://www.consumeraffairs.com/news04/2005/disability_insurance.html Basically, my theory is that I pay a DI provider. In return, they promise to give me money if I am disabled. Since I have no intention of being disabled, this is likely to be a good deal for them. However, if they have a history of reneging on that promise (as most do), their insurance is worthless to me regardless of how financially stable the company is. I don't know why anyone would buy insurance from a company that has a history of not paying.

I guess my question was not really clear, I am now covered for medical insurance, I need something that will help supplement/replace my income while I am unable to work. Does Medicaid have a program like that? Thanks again!

Can you collect disability for not being able to work due to bipolar complications? Hi. My girlfriend has just been diagnosed with Bipolar disorder (she was hospitalized). She was a home maker for several years in a previous marriage and hasn't been employed over the last few years. Additionally, she had no medical insurance, but is now getting help via medicaid. She is also pregnant... so working is tough. Can she collect disability in the state of Texas? How do you go about looking into this? Not sure where to start... Thank you for your answers!
CASDI vs. CAVPDI? I have a question for California residents or for anybody who knows about this. What is the difference between CA SDI (California State Disability Insurance) and CAVPDI (California Voluntary Plan Disability Insurance) ? This item usually appears on box 14, of W-2 form. I heard CASDI can be use for tax deduction, but CAVPDI can not . is this true ?

Are all the tax liabilities (Fed Taxes 941/944, etc) I paid for my business tax deductible on my 2007 taxes? I'm wondering if I can claim all of my tax liabilities for my business on my 2007 taxes. I think I messed up on my 2006 taxes and didn't claim all of the tax liabilities I should have. I didn't claim the amount I spent on Federal Taxes (941/944), Federal Unemployment (940), CA SUI/ETT and CA PIT/SDI (Income Tax and State Disability Insurance).
I reappled for long term & short term insurance at work and once again was denied. Is this LEGAL? A few days ago, I received my letter denying me long term & short term insurance coverage. The letter stated, "As you know, the insurance you are applying for is medically evaluated and certain health standards must be met to be eligible for enrollment. Another review has been made of the insurability applications you completed. Based on your condition of insulin dependant diabetes diagnosed 1989, it has been determined that our original decision to deny Short and Long Term Disability insurance coverage cannot be changed. Other pertinent health history includes high blood pressure and acid reflux." The letter goes on to state I have 90 days to appeal this decision. I must say I HAD this insurance and dropped it shortly after starting at The State of Texas. Does this mean I will never be eligible or should I fight this with an appeal? I can brag and say I don't call in sick (very often) and do a great job at work. I work 13 hour days and have a 2 year old son!!!!!!!!!
does my heavyduty job affect my ss disability chances? I was a sanitation worker for 21 years working behind garbage truck. I severly injured my right arm in snow storm on my job. I had surgery and 2 years of physical therapy of wich did not help! I have filed for social security disability and will wait a long time for a hearing. Being that I was a garbage man working behind garbage truck for 21 years will that help me get social security disability easier than lets say a truck driver or postal worker I was told by a state insurance rep. that my job as far as insurance purposes go is the most hazzerdous job in ny state! Thank you!

California disability after having baby.? I started working at a new job a few months ago where I only work 2.5 hours/day so that I could be home more when I had my daughter. I get paid a high hourly wage, and am considered to be a sub-contractor of a program that works with infants. They do not take any money out of my checks, not for taxes, SDI, or anything. I have the tax thing figured out, but am thinking of having another baby in the next few months and am trying to figure out if there is any way for me to get disability for the 6 weeks afterwards. Since I dont pay into SDI, I'm assuming I dont get state disability like I did last time. Am I correct in this? Is there any type of insurance I can purchase that would cover this? I live in California. Thank you. I dont know where some of you live, but here in California you get 6 weeks of disability for a vaginal delivery, and 8 weeks for a C-section. You get 4 weeks before you have the baby. I am surprised at some of the rude responses. I paid into SDI for several years before having a baby, so I deserved the 6 weeks of disability that I received after having my daughter. I have many friends who have had babies and they all received disability as well. To say to leave it to those who really need it is silly, since there a lot of people on disability because they didn't take care of themselves. Poor eating habits and lack of exercise causes many health problems and no one thinks that they shouldn't receive disability when they are taken off work.
