Insurance Plans & Infertility Coverage?

I live in CA and have a choice of 4 different insurance plans to choose from (through my employer) during the open enrollment period. The plans are: (1) United HeatlhCare (UHC) Choice Plus 1000 (2) PacifiCare HMO (3) Kaiser Permanente HMO (4) Blue Shield of California HMO I'm trying to find out which of these plans has the best coverage for infertility testing/diagnosis & treatment. Has anyone used or currently used any of the above insurance plans in this regard? Any place I can get additional information, short of calling each insurance company to find out? Thanks in advance!

1. The benefits will vary widely from contract to contract. You should request a benefits statement for each plan.
2. I think all of those choices have websites where you can search benefits. I have United and use a site called myuhc.com.
