Self Funded Insurance Plans?
My wife requires a procedure which is not covered by my corporation’s self funded health insurance plan. Is there a form I can file with the plans TPA or with my company itself for special cases to appeal for coverage? P.S. I know I need to work with my Insurance company! We have made numerous attemps calls, doctor recomendations, etc. I need to know if there is a formal process...

- You need to work with your insurance company.
- You have to work with the HR people in your company. You may have to pay for the surgery out of pocket yourself, if it is considered an elective surgery.
